Ok well I got bored and found this on the fellow interwebs. Sent it to my friends and am now making a collection of different peoples results.
If you'd fill one out and upload it it would be great as im trying to get and save as many as possible

Just fill it in with pictures and writing if needbe.

PS: Yes I'm aware there was a Zombie survival topic but I'm trying to get as many of these exact forms filled out, Plus It's not worth zombifying a topic (Pun not intended).
