Why do these things keep happening to me? (Same grunt after reloading to previous save. I really started to feel bad for the little guy.)

(For some reason the Hunters' blast missed, and almost all of the construction workers managed to survive. With barely any help from me - I gave them all better weapons and took out two brutes - they were able to take the hill at the end of the level.)

and this one elite recon it is a glitch on floodgate where you find an elite body and its a random permutation between the combat and the assault but sometimes it glitches and does both on one body.

Actually, the instruction is right. There is no Rocket there, it only indicates where to position your arms, not where to position the Rocket Launcher.
