Ok i read something about that you have to have the entire campaign done on normal which i have everything but the last level completed on normal so I'm going to try to beat it on normal tonight and see if the skull spawns after that

it worked the skull was there this time so apparently u need to have the whole game done on at least normal

With the catch Skull you need to have at least beat the game on Normal once for it to appear, I re-did this level over 5 times trying to get it but it wouldnt appear. Once I beat the game: bamf there it was.

I also heard that you can get it if you do Co-Op and have someone on the team who has beat the game at least once
